  • Acupuncture

  • Birth control

  • Breast pumps and lactation supplies

  • Chiropractor

  • Deductible costs

  • Dental care

  • Doctor’s office visits and co-pays

  • Eye exams

  • Fertility treatment (including in-vitro fertilization)

  • Glucose Monitors and supplies

  • Hearing aids and batteries

  • Inpatient hospitalization

  • Insulin
