1099-SA
You will be able to access your Form 1099-SA in the Documents tab, available only on our web portal: app.getbenepass.com on or before January 31st. There, you'll be able to download a PDF of this form directly.

Please note, this form is only generated for those who have distributions recorded in their HSA account from this past year. Distributions are any money movement out of your HSA account. If you have only contributed, you will not see this form on your account.

If you transferred your HSA to Benepass midway through the year, you may also be owed a form 1099-SA from your prior custodian.

5498-SA
Your Form 5498-SA is issued on or around May 31st. This form will provide an up-to-date summary of contributions to your HSA account for the prior tax year. Form 5498-SA is not required to file your taxes, and it will not be possible to generate until after tax day.
